Penn Medicine Princeton Hospice is seeking a Registered Nurse to join our team in New Jersey. This is a field based, full-time day shift opportunity. NJ Counties: Somerset, Mercer, Princeton, Middlesex (territories/counties assigned in accordance with need and proximity).The ideal candidate will be available to work 40 hours per week:¢ M-F 8:30AM- 5:00PM¢ Work 1 weekend per month (every 4th weekend) (Saturday + Sunday 8:30am-5:00pm)¢ Work 2 holidays per calendar year (1 winter + 1 summer)Case Load: 12-15 patients (on average 4 to 5 patient visits/day)Program Overview:Penn Medicine Princeton Hospice provides compassionate clinical care to patients with life-limiting illnesses, while supporting their loved ones. The team uses a holistic approach to meet the emotional and spiritual needs of both as they work through end-of-life issues. The team also supports patients efforts to live a full and meaningful life for as long as possible. Patients and their families choose hospice care when potentially curative or disease-modifying treatment for a terminal illness is no longer appropriate or desired. The decision to receive hospice care is made by the patient, in consultation with his or her personal physician and the caregiver.As a Clinical Nurse 2 with Penn Medicine Princeton Hospice, you will be providing end-of-life care to patients in their homes. Incumbent is responsible for taking a pro-active approach to pain and symptom management and recognizes that pain extends beyond the physical. As a Clinical Nurse in Hospice you will address the symptoms of each patient's illness as well as the emotional and spiritual concerns of the patient, caregiver and family assisting patients and loved ones with the challenges that come with death and dying. You must also be able to function effectively under stressful situations with tight deadlines; be a self-directed, self-starter; demonstrate creative thinking ability, as well as the knowledge and skills necessary to provide the expected standards of care; demonstrate the professional skill and judgment necessary to provide care within the autonomous setting of the patients home or at the various sites of hospice care; attend to the values and rituals important to the patient-family-community environments as well as upholding nursing practice and safety standards.Some job functions and duties will include, but are not limited to:¢ Utilizes data and QI methods to identify potential and actual problems and opportunities to provide care that is safe, timely, efficient, effective, and equitable.¢ Evaluates and integrates best current evidence with clinical expertise and patient and family preferences and values for the delivery of optimal health care and system effectiveness.¢ Collaborates and applies innovative, systems thinking to engage in systematic, evidence-based problem solving and decision making to promote effective changes within a complex care delivery system supporting the vision of Penn Medicine.¢ Recognizes the patient (or the patients designee) as the source of control and a full partner in providing compassionate and coordinated care based on respect for the patients preferences, values, and needs.¢ Demonstrates a commitment to the nursing profession through lifelong learning, adherence to ANAs Code of Ethics for Nurses, participation in a professional organization and advancing community outreach.¢ Minimizes risk of harm to patients, families, providers and self through system effectiveness and individual performance.¢ Utilizes appropriate information and technology to communicate, manage knowledge, mitigate error, and support decision making across the continuum.¢ Effectively engages in the process of cooperation, coordination, and collaboration in an effort to provide for safe, quality outcomes for patients within inter and intra-professional teams, including virtual teams.¢ Performs duties in accordance with Penn Medicine and entity values, policies, and procedures¢ Other duties as assigned to support the unit, department, entity, and health system organization

•\tMust have Bachelor's Degree in Nursing (BSN) from an accredited school of nursing, at time of hire.\n•\tMust have more than 15 months relevant professional nursing experience and completion of the Nurse Residency Program, if applicable, at time of hire.\n•\tMust possess active Pennsylvania RN License, at time of hire.\n•\tMust possess active BLS/CPR, as a healthcare provider as per the American Heart Association at time of hire.\n•\tMust have a valid driver’s license, a car and car insurance.
