Penn Medicine Princeton Hospice is seeking a per diem Licensed Practical Nurse to join our team in New Jersey!Program Overview:Penn Medicine Princeton Hospice provides compassionate clinical care to patients with life-limiting illnesses, while supporting their loved ones. The team uses a holistic approach to meet the emotional and spiritual needs of both as they work through end-of-life issues. The team also supports patients efforts to live a full and meaningful life for as long as possible. Patients and their families choose hospice care when potentially curative or disease-modifying treatment for a terminal illness is no longer appropriate or desired. The decision to receive hospice care is made by the patient, in consultation with his or her personal physician and the caregiver.Position Summary:As a Licensed Practical Nurse (LPN) with the Penn Medicine Princeton Hospice team, you will provide care and health services to patients by following the Plan of Care and instructions from the Registered Nurse Case Manager. Care, education, and treatments will be provided in the patient's residence which may be a home, facility or nursing home. The LPN must function with a large degree of independence and draw on their education and experience to provide care and treatment in a variety of conditions. A hospice LPN may also be scheduled to provide 1:1 continuous care for hospice patients who are actively dying and need skilled services. The LPN serves as part of the interdisciplinary team and will participate in team conferences to show observations, interactions, and barrier to meet goals.Job responsibilities will include, but are not limited to:¢ Provides home health services to patients through regularly scheduled home visits.¢ Follows established Plan of Care (POC) developed by the RN and understand the process for changing the POC according to the patient and family needs.¢ Performs approved nursing procedures and treatments within the LPN scope of practice. Follows appropriate preventative and palliative nursing procedures according to the Nurse Practice Act for LPNs.¢ Documents all interventions, patient/family response, and medication administered in the medical record. Maintains patient records so that problems, plans, actions, and goals are accurately and clearly stated. Completes patient notes the same day that patient is seen or shift care provided.¢ Assists in the planning and evaluation of the nursing care for patients and their families.¢ Reviews POC with the RN and RN Team Manager. Updates and revises the POC under the direction of the RN to address current patient and family needs.¢ Participates in performance improvement activities as requested.¢ Performs duties in accordance with Penn Medicine and entity values, policies, and procedures¢ Other duties as assigned to support the unit, department, entity, and health system organization

Educational Requirements:\n• High School Diploma/GED\n• Graduate of an NLN accredited licensed practical nursing program\n\nLicensure, Certification or Registration Requirements:\n• Pennsylvania and/or New Jersey LPN licensure depending on geographic location and scope of responsibilities.\n• BLS/CPR Certification\n• Valid Driver’s License\n\n\nRelated Work Experience:\n• Full time experience as a Licensed Practical Nurse\n• 2+ years Acute care, home care, hospice care, or long-term care experience.\n\nSpecialized Skills & Knowledge Requirements\n• Must be able to satisfactorily complete the orientation process and competency testing.\n• Must have demonstrated knowledge of palliative care and home health nursing care.\n• Must be able to communicate to families and patients to reinforce care principles.\n• Must be able to visit patients in various geographic areas as determined by agency contracts.
