The Admission RN is responsible for the entry of patients and families into the spectrum of services provided by Wissahickon Hospice. As a member of the Interdisciplinary Team (IDT), the Admission RN provides information to potential hospice patients, assists with the decision making process, performs assessments that assure the appropriate level of care is delivered, and expedites entry into service delivery systems. The professional nurse must attend to the values and rituals important to the patient-family-community environments as well as upholding nursing practice and safety standards. In addition, the professional nurse is responsible for practicing within the Hospice regulations and reimbursement criteria. + As such, the nurse is responsible for determining the skilled nursing care and visit pattern to maintain quality and meet the patient and familys goals. The professional nurse has a working knowledge and understanding of hospice, the interdisciplinary team concept and the role of the Admission RN. The Admission RN functions within the interdisciplinary team to facilitate communication to promote and support symptom and disease management. The Admission RN demonstrates safe practice within the UPHS professional practice model and demonstrates UPHS core values which reflect its mission. The Admission RN provides direct care to adult patients and demonstrates safe practice within the domains of clinical judgment and decision-making, nurse-patient/family relationship, clinical leadership, and clinical scholarship. The Admission RN works closely with others on the healthcare team to facilitate the coordination of care across the continuum. The nurse demonstrates the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age appropriate considerations of assigned patients. The nurse complies with all regulations and standards of regulatory and accrediting bodies. The Clinical Judgment and Decision Making skills of the Admission RN are characterized by safe clinical judgment in the delivery of patient care. The Admission RN reviews documentation of potential clients who are referred to hospice. The Admission RN arranges for initial contact in a timely fashion and communicates plans for contact with client and family and referral sources. The Admission RN demonstrates the ability to organize and prioritize the workload, recognize emergency situations, and appropriately seek assistance from the healthcare team. The Admission RN establishes a professional relationship with the patient and family. The Admission RN instructs primary caregiver of immediate care needed to be provided and informs primary nurse of the initiation of the plan of care. The Admission RN demonstrates Clinical Leadership skills by exhibiting professional behavior, utilizing appropriate resources and demonstrating flexibility. The Admission RN utilizes practice standards and guidelines. The Admission RN is a responsible team member who works with others in a respectful manner with sensitivity to cultural and role differences. The Admission RN supports colleagues in their professional development and maintains open communication. The Admission RN demonstrates Clinical Scholarship by identifying learning needs, seeking work experience which strengthens skill acquisition and seeking assistance from appropriate healthcare team members. The Admission RN must complete all Admission RN competencies within 15-24 months and apply for the next clinical level position.

Minimum Job Requirements: \nGraduate of an NLN or CCNE approved school of professional nursing. \nLicensed to practice professional nursing in the Commonwealth of Pennsylvania. Less than one year work experience as a professional nurse. A BSN from an accredited program is required for all new RNs hired. \nMust have Basic Life Support CPR certification, have a car, hold a valid driver’s license, and have car insurance. \nResponsible for visiting patients in various geographic areas as determined by agency. \n1 year Hospice and Home Care experience required.
