Penn Medicine Princeton Health is looking for an Acute Care Nurse Practitioner for the Princeton Medicine Hospitalist ServiceThe Nurse Practitioner provides direct and indirect patient care, admission support, and consult services in collaboration with the physicians in the acute inpatient setting and the Observation unit at Penn Medicine Princeton Medical Center.Additional Responsibilities include: - Independently evaluate patients requiring admission to either the inpatient or observation unit. - Promotes objective and timely evaluation basedon set criteria and clinical acumen.Performs physical examination and documents findings.- Independently assesses and follows patient and documents admission note, progress note, discharge note. Orders appropriate medications,laboratory assessments and imaging in collaboration with the Attending physician.- Communicates findings and care plan with patient, family and others in the care team.- Reviews patient history with the patient and family and PC P and /or specialists, as detailed in unit protocol and best practice. Obtains andreviews prior records from recent hospitalizations and/or outpatient visits, and coordinates discharge planning and ensures proper transition ofcare. Communicates discharge/transfer information to appropriate providers. Perform patient and family education as needed.
Advanced Nurse Practice Degree from an accredited nursing school and all of the education required to have achieved such a degree. Active NP licensure in the State of NJ.\nAcute care track preferred. Board certification as appropriate.
